CO-RADS ( COVID 19 REPORTING & DATA SYSTEM )

📙 CO-RADS
means COVID 19 REPORTING AND DATA SYSTEM
  • It is used as a standardized diagnostic tool for assessing pulmonary involvement in COVID-19 patient & to categorize it based on unenhanced chest CT / HRCT (Computed Tomography) scan findings.
  • HRCT ( High resolution CT ) scan has a HIGH SENSITIVITY & HIGH SPECIFICITY values.
  • So, False Positive & False Negative rates are low.

🌏CO-RADS 0 :

  • Level of suspicion for pulmonary involvement of COVID-19 is : NOT INTERPRETABLE
  • Summary : Scan technically insufficient for assigning a score.
  • Scans are incomplete or of insufficient quality, for example because of severe artifacts due to coughing or breathing.

🌏CO-RADS 1 :

  • Level of suspicion for pulmonary involvement of COVID-19 is : VERY LOW
  • Summary : NORMAL OR NON INFECTIOUS
  • This category is identical to the “negative for pneumonia”

🌖CO-RADS 2 :

  • Level of suspicion for pulmonary involvement of COVID-19 is : LOW
  • Summary : Features are typical for OTHER INFECTION and it is NOT compatible with COVID 19
  • Examples includes Bronchitis, Infectious bronchiolitis, Broncho pneumonia, Lobar pneumonia, and Pulmonary abscess

🌗CO-RADS 3 :

  • Level of suspicion for pulmonary involvement of COVID-19 is : EQUIVOCAL / UNSURE
  • Summary :  Features compatible with COVID-19 but also with other diseases.
  • Findings include :
  • equivocal findings for pulmonary involvement of COVID-19 on CT scan which is also found in other viral pneumonia or noninfectious causes.

🌒CO-RADS 4 :

  • Level of suspicion for pulmonary involvement of COVID-19 is : HIGH
  • Summary : Suspicious for COVID 19
  • Findings include :
  • Perihilar ground-glass opacity,
  • Homogenous extensive ground-glass opacity
  • Interlobular septal thickening with or without pleural effusion in the absence of other typical CT findings.

🌑CO-RADS 5 :
  • Level of suspicion for pulmonary involvement of COVID-19 is : VERY HIGH
  • Summary : Typical for COVID 19
  • Findings include :
  • Ground-glass opacities with or without consolidations
  • Crazy paving pattern
  • Reverse halo signs
  • Bands of ground glass opacities

🎇CO-RADS 6 :
  • Level of suspicion for pulmonary involvement of COVID-19 is : PROVEN CASE OF COVID 19
  • Summary : RT PCR Positive for SARS CoV2
  • Patient is having positive RT-PCR test results for virus-specific nucleic acid detection test.
  • we are doing this test for assessing Percentage of pulmonary involvement & for Prognostic purpose in these patients.

Mucormycosis simplified notes.

Welcome to DRD medshortnote.   Today we will discuss about MUCORMYCOSIS :- This topic contains :- Microbiology of mucormycosis Pathophyiology of disease caused by it Clinical features  Diagnosis and investigations  Management 1) MUCORMYCOSIS MICROBIOLOGY : Mucormycosis is caused by Fungi - order Mucorales of subphylum mucormycotina. All Fungi are classified morphologically into 4 major classes :- Yeasts Yeast like Molds Dimorphic Mucorales comes under MOLDS ( ASEPTATE MOLDS ) mucorales includes : Rhizopus ( most common cause) { R.oryzae & R. delemar } Mucor  Rhizo mucor  Cunninghamella Lichtheimia   image showing aseptate broad hyphae seen under microscope in gomori methenemine silver stain.  schematic diagram of asexual form of Rhizopus fungi 2) PATHOPHYSIOLOGY OF MUCORMYCOSIS : - Mucorales are ubiquitous organism but it is present mainly in humid atmosphere, dust, in kitchen garbage, in soil, on rotten bread, etc. Causes infection in - Diabetic...

Pharmacology of PARACETAMOL & Acute Paracetamol poisoning.

Welcome to DRD- medsortrnote. Today we will discuss about Paracetamol.      This topic includes :- General properties & Mechanism of action Dosage Use Adverse reactions Toxicity, Clinical features & How to Manage Paracetamol toxicity  1) GENERAL PROPERTIES AND MECHANISM OF ACTION :- General properties:- Paracetamol / N-acetyl P-aminophenol / Acetaaminophene is classified under group of drugs called Para-aminophenol derivatives under Major group called NSAIDs Non Steroidal Anti Inflammatory Drugs. Acetanilide is the parent member of this group of drug. it was first used in medicine by Von Mering in 1893. Peak plasma concentration Cp = In ½ to 1 hour.   t ½ is 2hours Protein binding is 20 to 50% Metabolized by Glucuronides (60%) and Sulfates (35%) Mechanism of Action :-  It has both Anti Pyretic and Anti Inflammatory activity. but Anti pyretic effect is more pronounced. M.O.A.:- weak Nonspecific COX (Cyclooxygenase) enzyme inhibition by...

ART ( ANTI RETROVIRAL THERAPY ) regimen in children (simplified)

📙 BASIC RULES IN A.R.T. ( Anti Retroviral Therapy ) for children; according to NACO ( National Aids Control Organisation ) guidelines :  ❶ In child <3 years EFV ( Efavirenz ) is not given. So, it is replaced by LPV/r Lopinavir boosted combination. ❷ In <10 years old child TDF ( Tenofovir ) is not given . So, it is replaced by ABC ( Abacavir ) ❸ In case of Anemia - AZT ( Zidovudin ) is not given. So, it is replaced by ABC ( Abacavir ) ❹ In case of Child with Renal disease - TDF ( Tenofovir ) is not given. So it is replaced with ABC ( Abacavir)   📙 Basic formula is : 3TC + TDF + EFV 3TC = Lamivudin TDF = Tenofovir EFV = Efavirenz AZT = Zidovudin ABC = Abacavir LPV/r = Lopinavir ( ritonavir boosted )       ① ART REGIMEN IN CHILD <3 YEARS AGE :- EFV replaced with LPV/r TDF replaced with AZT In Anemia AZT is replaced with ABC AGE (Years)   <3 years     ...
